DISASTER STRIKES: THE CRASH OF VALUJET FLIGHT 592<br />
On June 10, 1995, <a class='nonexistent' href='/wiki/wiki.php/ValuJet'>V</a>aluJet Flight 597 was taxiing in<br />
preparation for takeoff when one of the DC-9s fuselage-<br />
mounted engines burst into flames. Pieces of a compres-<br />
sor disk penetrated the passenger cabin and seven people<br />
were injured in the incident. The Federal Aviation Com-<br />
mission carefully studied the incident. FAC officials an-<br />
nounced that they had observed no shortfalls in <a class='nonexistent' href='/wiki/wiki.php/ValuJets'>V</a>aluJets<br />
maintenance practices, and the airline continued busi-<br />
ness as usual. Almost a year later, the airline was in the<br />
news again, and this time for a much more serious failure<br />
that would have far-reaching consequences.<br />
On May 11, 1996, <a class='nonexistent' href='/wiki/wiki.php/ValuJet'>V</a>aluJet Flight 592 crashed into the<br />
Florida Everglades, killing all 110 people aboard. Investi-<br />
gators determined that the DC-9 was carrying 119 suppos-<br />
edly empty oxygen generators in its forward cargo hold.<br />
The generators are commonly used on passenger aircraft<br />
to produce oxygen for air masks.330 However, <a class='nonexistent' href='/wiki/wiki.php/ValuJet'>V</a>aluJet was<br />
not authorized to carry the generators as cargo.331 During<br />
the flight, oxygen gas was released in the cargo area,<br />
generating heat in excess of 500F that created an explo-<br />
sion sending the plane into a fiery crash. None of the<br />
oxygen generators carried by Flight 592 had safety caps,<br />
which could have prevented them from heating up due to<br />
turbulence during the flight. <a class='nonexistent' href='/wiki/wiki.php/ValuJet'>V</a>aluJet was shipping the<br />
generators from a Florida maintenance shop to Atlanta for<br />
refurbishment. Following the crash, the airline contended<br />
the canisters were indeed mislabeled empty by one of its<br />
maintenance subcontractors.332<br />
Even before the crash occurred the Federal Aviation<br />
Administration (FAA) was investigating <a class='nonexistent' href='/wiki/wiki.php/ValuJet'>V</a>aluJet. A 120-<br />
day re-certification inquiry had been prompted by an<br />
internal memo dated February 1996 that stated that 20<br />
of the 48 violations <a class='nonexistent' href='/wiki/wiki.php/ValuJet'>V</a>aluJet had incurred since 1993 were<br />
still unresolved. In the wake of the Everglades crash, the<br />
